Variations of these forms to suit the user's needs are allowable. It is recommended that the qualification and NDT information required by this code be recorded on these forms or similar forms which have been prepared by the user. Also included is a certificate of conformance and laboratory report forms for recording the results of NDT of welds. It is recommended that the qualification and NDT information required by this code be recorded on these forms or similar forms which have been prepared by the user.

Form Description N.1 N.2 N.3 N.4 N.5 N.6 N.7 AWS D1.6 Code Forms These sample forms have been approved by the AWS D1 Committee on Structural Welding for the recording of procedure and performance qualification required by the AWS D1.6 specification. It is recommended that the qualification information as well as essential variables required by this specification be recorded on these forms or similar forms which have been prepared by the user. Variations of these forms to suit the user's needs are allowable. Form Description M-1 M-2 M-3 AWS B2.1 Code Forms These sample forms have been approved by the AWS B2 Committee on Procedure and Performance Qualification for the recording of WPS qualification, welder qualification, and welding operator qualification data required by the AWS B2.1 specification.
